Waiver of Liability and Hold Harmless Agreement




In consideration of attending the Wellesley Chinese Language School (WCLS), I, as a parent of my guardians (children), hereby acknowledge that the school is a non-profit community service-based organization, and, accordingly, I hereby knowingly and voluntarily release, waive, discharge, and covenant not to sue WCLS, its officers, teachers, or employees (hereinafter referred to as “School Parties”) from any and all claims, demands for compensation, actions and causes of action of any kind, in law or in equity, or arising out of any and all known or unknown, foreseen or unforeseen bodily or personal injuries, property damage or the consequence thereof, resulting from any accident, casualty, or event involving WCLS.

I, as a parent of my guardians (children), will look to our own resources and insurance, if any, in the event of any injury or other occurrence, of whatever description, resulting in connection with any events occurred in during WCLS school time.


Furthermore, I  hereby agree to indemnify and hold harmless the School Parties from any loss, liability, damage or costs, including court costs and attorneys fees, that they may incur due to the use of the school space (Village Church, a rented space to WCLS), whether caused by the negligence of the School Parties or otherwise.


I understand that the School Parties does not waive liability for intentional or reckless acts, as such claims cannot be waived, but do understand that I will waive claims of any other kind, specifically including claims for negligence, against the School Parties.


In submitting this form, I acknowledge and represent that I have read this Waiver of Liability and Hold Harmless Agreement, understand it, and sign it voluntarily as my own free act and deed on as a parent; no oral representations, statements, or inducements, apart from the foregoing written agreement, have been made; and I execute this form for full, adequate, and complete consideration, intending that the I will be bound by it.

Consent Agreement to the Parent Responsibilities and Rules


To ensure a productive and safe learning environment at the Wellesley Chinese Language School (WCLS), all parents/guardians have the ultimate responsibility to follow the parent responsibilities and rules issued by the school. All parents/guardians will be required to understand and agree to the Parent Responsibilities and Rules of the Wellesley Chinese Language School (WCLS) before registering classes in WCLS.

  1. All parents/guardians voluntarily send their child(ren) to WCLS and waive all liability claims against the school and its personnel.
  2. Parents/guardians take full responsibility for the safety of themselves and all the family members while attending WCLS. Parents/guardians should accompany their child(ren) to ensure their child(ren) arrive at WCLS safely. Parents/guardians/students will not hold WCLS liable for any personal, bodily injury or any personal property damage, which may occur in the premise during school time.
  3. Parents/guardians should remind their child(ren) not to use, move or modify any classroom items that belong to the Church, including but not limited to books, toys, etc. Parents/guardians are liable to any and all property damage caused by them and their child(ren).
  4. Parents/guardians should educate their child(ren) to obey the Student Rules of WCLS rules during school time. Parents/guardians should cooperate with teachers to address any concerns regarding their child(ren)’s behavior at school. No running or shouting at school premise. Food is not allowed in the classrooms.
  5. Parents/guardians are responsible for providing voluntary service to WCLS coordinated by the WCLS school parent committee during school operation time and agree to pay $50 to waive the parent duty responsibility.
  6. Parents/guardians should follow the school emergency plan.
  7. Parents/guardians should remind their child(ren) not to use the first floor of the Church during school time.
  8. Parents/guardians are responsible for their child(ren)’s Chinese study at home. Parents/guardians should encourage, help and monitor their child(ren) to complete homework, test preparation and other related activities. In case of confusion, parents/guardians should proactively contact a teacher to clarify issues.
